Initial Therapy Paperwork

These documents must be received to our office prior to your first therapy appointment. We will not be able to see patients until this paperwork is complete. Please return these in advance or arrive at least 20 minutes early to complete on the day of your first visit.

• Patient Initial Treatment Packet - DTA Kennesaw Office: to be completed prior to evaluation or treatment- can be completed on-line and emailed or printed and delivered to office.
• Physician Prescription (PDF) for Services (call us to confirm the services you should request for your specific needs)
• Medicaid/Insurance cards (front and back; please confirm your coverage or payment plan options with us BEFORE your first appointment)

AAC/Speech Generating Device Evaluation Paperwork

Note: We will need a copy of the current IEP for all therapy patients who are receiving services through Medicaid.

Please talk with us before your first appointment about our costs, your insurance/Medicaid coverage and payment plans, if private payment is necessary, to ensure you understand any personal financial obligations.

• We are in-network with several major insurance companies including: Blue Cross Blue Shield HMO, POS, and PPO, Aetna, and United Healthcare but...
• Most insurance companies deny developmental speech language therapy! Although you, and we, can call and confirm coverage, we have experienced a significant amount of ambiguity with the information from insurance companies. Confirmed coverage from your insurance, either verbal or in writing, in no way guarantees their payment. The only guarantee we have of coverage is their response to our direct bill. Although we will bill insurance for every claim, you are responsible for any bills unpaid by your insurance company. We bill immediately after services but it can take 30-60 days for the insurance company to determine their payment.
• We offer a discount for private paying patients. If your insurance company doesn't cover your costs, we will bill you at the discounted rate.
• We are a provider for Georgia Medicaid. We can accept Medicaid for patients who have SSI Disabilities Medicaid or the Deeming Waiver ("Katie Beckett Waiver").
• We cannot accept any patients who have coverage under a Medicaid CMO which includes Peachstate, Wellcare, and Amerigroup.
